To access the form you need, simply click on the appropriate button below. The first 12 buttons will take you immediately to the online form. Simply fill it out with all the applicable information and hit submit at the bottom of the page.

The two forms in the "Special Instructions" section are not available as web forms. Please click on the buttons for those forms to go to a page with the instructions you need to complete and submit the forms.


To help prevent submission errors, please follow these instructions.
  1. SAVE THE FORM FIRST! As soon as you download the form, save a copy to your computer.
  2. SAVE AND SAVE OFTEN! While you are completing the forms, please save the form every couple of minutes to be sure you don't accidentally close the form and lose your data. Given that the forms are .PDF files, when you make changes and try to close the form, it will likely prompt you to save again before you close it, but its a good standard practice to actively save the form along the way. Remember, Jesus saves, and so should you!
  3. SAVE BEFORE YOU SUBMIT! Prior to submitting the form, make sure you save the form again!
Since this years forms are in a .PDF, you can print and save the forms as often as you'd like while preserving the information in the form.  We've found that several congregations typically print, then share the printed form(s) with the appropriate parties in your congregation, have them fill out the form on paper and return it to the church office for submission. We recognize within our conference, there are myriad ministry settings with varied levels of administrative support. However, we've found that doing the forms this way makes for a smoother submission process.
