Beginning June 2012, the health insurance apportionment will be discontinued and we will move to direct billing for the total health insurance premium. When completing the 2012 Pastor Support and Compensation Form (PSCF), the amounts reported on Lines B and D.1.a. will remain the same in 2012 as reported in 2011 (if there is no plan change or the cost sharing arrangement has not changed). The remainder of the total premium for the year goes on Line G at the bottom of the PSCF. Line G is for information only and will reflect the difference between the totals of Line B plus Line D.1.a. and the total premium amount for 2012 (the additional amount that is no longer apportioned).
